PREMIER makes every effort to participate with as many insurance companies as possible. If your insurance company currently participates with us, you are responsible for the copay that your plan dictates at the time of service. In most cases, you will be responsible for either a specialist or urgent care copay and certain plans require an emergency room copay. For some insurance carriers, there may be additional charges above the copay depending on the patient's individual benefit plan.
For your specific copay, co-insurance, or deductible, please check with your benefits manager, or call one of our centers and someone will be able to assist you. Due to the variation in plans, our staff can not answer exact amounts that the patient will owe. The final out of pocket cost due from a patient is dictated by the insurance carrier.
