If your insurance company currently does not participate with us, please let us know (on the contact page) so we can make the appropriate changes to your file, and so we can begin the application process with your company.

You may still be seen at our center, and we will provide you with the appropriate paperwork to return to your insurance carrier for reimbursement. Payment of fees will be due in accordance with our payment policy.
